PREPARATION OF SAMPLES

Our Team is always pleased to help.

For genetic examination nuclei-containing cells are required, which are either cultivated or from which DNA is extracted. Therefore, no special preparation of the patient is required, blood can be collected at any time of the day and should be sent directly to the laboratory at room temperature or slightly cooled (8-20 °C).

Documents

Identification of samples and request more information

For clear identification please label sample tubes either with barcodes or with the name and the date of birth of the patient.
Please use our laboratory request forms. A detailed medical record of the patient as well as family history will significantly support our experts in designing the diagnostic and for verification of the diagnosis.

Informed consent  more information

For each genetic analysis an informed consent is required due to German law. You may use our standard form for an informed consent for molecular- and cytogenetic analyses.
For Array-CGH and kinship analyses respectively more detailed consent forms are necessary. Please, use for Array-CGH analyses the consent form Array-CGH analyses: informed consent Array-CGH. Please, fill in the complete form for kinship analysis including the consent for kinship analyses and return the complete form to us.

International Transmittal more information

Outpatient or patients of physician in private practice in European countries

First of all a request for meeting the costs has to be accepted by the responsible health insurance. To process this request the form E-112 has to be filled in (Form can be ordered from health insurance).

It is recommended to enclose a short justification of the medical necessity of the specific analysis. As soon as meeting the costs is ensured by the health insurance, please, send the following documents together with the patient’s sample to synlab MVZ Humane Genetik:

  • E-112-form
  • Request form
  • Informed consent

Inpatients from Austria and Switzerland

For inpatients from Austria and Switzerland we would issue a form for cost absorption by the hospital for the requested analyses. Either we issue the form after sample receipt or you may ask for this form prior to sample transmission. As soon as the hospital’s declaration of cost absorption is confirmed the signed form should be returned to synlab MVZ Humane Genetik.

Further international patients

International patients who cannot make use of the form E-112 or are from outside Europe we are working with prepayment. Please send in the sample material and both the request form and informed consent.

Immediately after receiving the sample we issue an invoice which is sent to the recipient of the invoice as indicated on the request form. We would also be pleased to  send the invoice per e-mail or fax to ensure quick processing. After the accounts are settled the analysis will be started. We accept payment by bank transfer as well as payment by credit card. For credit card payment, please, see our form “Terms of Payment”.
